The templates have been updated and added to and have been made available from 1st July 2025. These templates aim to assist applicants and members develop their plant biosecurity systems and ensure they align with the Plant Health Management Standard - version 1.3. These templates have been set out and reviewed by the Technical Advisory Group.
Only some of the requirements currently have templates available - we will aim to develop more templates if and as required. The templates that we have set out will be improved and updated periodically with a view to assisting businesses and organisations develop an effective plant biosecurity system for their site and operations.

The following updates and additions have been made to the templates (from the previous versions):
- All templates_ add N.B. that a member / applicant may use their own recording system/ template.
- 2.1_Plant biosecurity policy (i) Add signature for the most senior member of staff to sign and cover CPD in staff training.
- 4.1_SOPRA (i) Add pest management sheets as links (ii) update mitigated risk ratings (iii) remove two reference tabs that related to previous risk ratings. N.B. With change to mitigated risk ratings of 60 and above, the template now covers 36 pests (22 priority pests plus 14 with a mitigated risk rating of 60 or above). Reduced from 39 pests in previous version.
- 5.1_Supply chain management (i) include FC (or equivalent) / FRM newsletter as examples of info sources (ii) ask if supplier keeps a record of all notifiable pests detected on their site or associated operations (iii) add two new rows that distinguish between plant formats and other plant products.
- 7.1_Goods-in (i) add footnote with reference to PHMS 1.5 and the new PHMS 8.4 requirement.
- 7.3_Plant protection treatments (i) add prompt to link to individual crops / batches (ii) add target pest(s) and aim of action / treatment (iii) add prompt to treatment to detail area covered and dosage.
- 7.4_Dispatch (i) no update other than the N.B. change to all (above).
- 7.5_Complaints template (i) add ‘(specify)’ in parenthesis to ‘other’ column under issue type.
- 8.1_Crop, plant or area monitoring (i) edit to title – addition of ‘plant or area’ (ii) and species, cultivar, variety in parenthesis to ‘plant name’ column (iii) add a column to capture quantity of the plants inspected.
- 8.3_Continual improvement (i) new template to support members to record areas of improvement.
- 8.4_Pest detection and RCA (i) new template to record notifiable pest detections / incursions /outbreaks and a Root Cause Analysis (RCA) process to identify and address the underlying cause.
